A $25 deposit is required when booking Initial Evaluations.
Cancellations made less than 48 hours before your scheduled appointment will result in a $25 cancellation fee.

Holland
Dr. Nicole sees patients at her office within Rooted Family Chiropractic in Holland on Wednesdays and Thursdays.
Frankenmuth
Hannah, MSOT, OTRL sees patients at her office within Sage Wellness Center in Frankenmuth on Mondays and Tuesdays.
Your Home
If you live on the West Michigan Lakeshore within 20 miles of West Olive, we can come to you for in-home pelvic floor physical therapy appointments on Mondays!
