Please note:
A $25 deposit is required when booking Initial Evaluations, whether at home or in the office.
Cancellations made less than 48 hours before the scheduled appointment will result in a $25 fee charged to the card on file.

Dr. Nicole sees patients at her office within Rooted Family Chiropractic in Holland on Wednesdays and Thursdays.
Frankenmuth
Hannah, MSOT, OTRL sees patients at her office within Sage Wellness Center in Frankenmuth on Mondays and Tuesdays.

If you live on the West Michigan Lakeshore within 20 miles of West Olive, we can come to you for in-home pelvic floor physical therapy appointments on Mondays!
